Patients who come to UCLA for cancer care benefit from the most advanced forms of therapy available and from a variety of supportive services. Access to the latest protocols for investigative treatment of cancer and hematologic malignancies are available at UCLA’s cancer clinic. UCLA has been recognized for its exemplary interdisciplinary programs in cancer research, patient care, professional education and community outreach in cancer preventions. Phase I and Phase II clinical trials of new therapies as well as Phase III National Cancer Institute sponsored clinical trials are available for many different cancer diagnoses. Teams of specialists work together to help cancer patients and their families. Our multidisciplinary approach to assist patients and family members contend with both immediate and long-term effects of this disease. Convenient, service-oriented, state-of-the-art outpatient care is provided at the UCLA-Santa Monica Medical Center and the UCLA Medical Center provides integrated continuity of care with a complete range of medical, surgical, and supportive services. The UCLA Hematology-Oncology Program provides both investigative and standard of care therapy for patients with hematologic and oncologic diseases. Diagnostic and therapeutic consultations are also available. An integral part of the comprehensive care offered at UCLA is education, information, and support for you and family members. Your First AppointmentIf you are a first-time patient, please plan on arriving 30 minutes early in order to complete a medical history form and registration documents.
Please bring the following items to the appointment: Insurance card(s) Authorization or referral forms: if you have an HMO or POS plan, you will be asked for a copy of the authorization for the patient to be seen at UCLA. Failure to bring an authorization for your visit will necessitate payment in full for the visit Any required co-payment Pertinent medical records from your referring physician: Copies of any radiology films (x-rays, CT scans, MRIs, etc) Copies of any pathology reports with blocs or slides if possible All of the above items will be maintained on file at UCLA. If you need the pathology blocs or slides returned, please let your physician know.

For return visits, be sure to let the reception desk know if you have moved, changed your phone number, or changed your insurance. Be sure to pay your co-payment upon check-in. Click here for New Patient Registration Information Forms Billing and FeesThe fee quoted to you at the time you made your appointment is your physician’s new patient consultation fee. This fee, which is for physician’s services only, is billed through the UCLA Department of Medicine Professional Group. The billing office will bill primary and secondary health plans directly. Please make sure we have accurate insurance information so this can be done correctly.
Fees for additional services such as laboratory tests and x-rays, UCLA review of outside radiology films, or pathology blocs or slides will be discussed with you if these services should become necessary for your care. If these services should be needed, they will be billed separately through the UCLA Medical Center.
Patients whose healthcare coverage requires a co-payment in any amount will be asked for payment at the time of the appointment.
Patients with any form of managed care (HMO) coverage should not that the initial consultation will be limited to services authorized by their plan as part of the initial consultation. We will request authorization for additional services requested by the attending physician, but UCLA Hematology-Oncology cannot request authorization for the initial consultation itself. That request must be made by the patient or by the referring physician. If you do not bring an authorization for the initial consultation, you will be asked to pay the consultation fee or you may reschedule your appointment to a later day when authorization is available.
Following your initial consultation, all other services will be based on your physician’s orders. These additional services are subject to authorization before they can be rendered. Please be advised that not all insurance companies will allow care at UCLA. The staff at the Hematology-Oncology Center will inform you if there are problems getting authorization or if portions of your care must be completed at other facilities, selected by your insurance provider.
If you do not have healthcare coverage, payment of the physician’s fee is expected at the time of your appointment. Visa, Mastercard, Discover, and American Express credit cards are accepted, in addition to cash and personal checks. Additional insurance information.
